When a tooth cannot be saved

Tooth extraction

Sometimes a tooth is too damaged or loose to save. The dentist will first check whether it can be repaired and explain every option.

What is a tooth extraction?

A tooth extraction is the removal of a tooth from its socket in the jawbone. A dentist may suggest it when a tooth is too badly decayed, broken, or loose to save. Dr. Wael Sadek will first look at whether the tooth can be repaired. He will explain your options and what each one means before any tooth is removed.

Keeping your natural tooth is usually the first goal. When that is not possible, removing the tooth can stop pain and infection and protect the teeth around it.

Signs you may need an extraction

  • A tooth has decay so deep that it cannot be repaired with a filling, crown, or root canal.
  • A tooth is cracked or broken below the gumline.
  • Advanced gum disease has made a tooth very loose.
  • An infection keeps coming back even after other treatment.
  • A baby tooth will not come out and is blocking an adult tooth.

Wisdom teeth: [OFFICE TO CONFIRM: whether wisdom teeth removal is done in the office] — call and ask, and if a tooth needs care we do not provide here, we will tell you where to go.

What happens at the visit

  1. Exam and imaging. The dentist looks at the tooth and uses digital imaging to see the roots and the bone around them.
  2. Health review. Tell us about all your medicines, especially blood thinners, and any health conditions. These can change how the visit is planned.
  3. Numbing. The area is numbed so you feel pressure, not sharp pain.
  4. Removal. The dentist gently loosens the tooth and lifts it out. You will feel pushing and pressure during this step.
  5. Gauze and instructions. You bite on gauze to help a blood clot form. We go over your aftercare instructions before you leave.

Aftercare and healing

The blood clot in the socket protects the bone while it heals. For the first few days, do not use straws, spit forcefully, or smoke. These can loosen the clot and cause a painful problem called dry socket.

  • Bite on gauze as directed until the bleeding slows.
  • Eat soft foods and chew on the other side.
  • After the first day, rinse gently with warm salt water if the dentist tells you to.
  • Take pain medicine only as directed by the dentist or the label.

Call us if bleeding does not slow down, or if pain gets worse a few days after the extraction instead of better. If you have pain, call as early in the day as you can and the front desk will look for the soonest time open. If you have facial swelling that spreads toward your eye or neck, a fever, or trouble breathing or swallowing, call 911 or go to an emergency room.

Replacing a missing tooth

A gap can let nearby teeth drift and change your bite. Ways to fill the space include a bridge or dentures. Ask us whether a dental implant is an option for you. The dentist will explain the choices that fit your mouth.

Cost factors

The cost depends on the tooth, how its roots sit in the bone, whether imaging is needed, whether you plan to replace the tooth, and your insurance plan. We accept dental insurance, so call to check your plan. CareCredit can spread larger treatment into monthly payments. Ask for an estimate before treatment. See our insurance and payment options.

Questions patients ask

Will I be awake during a tooth extraction?

Yes. The area is numbed with local anesthetic, so you stay awake and feel pressure rather than sharp pain. If you feel nervous about dental work, tell us when you call. Ask us whether other comfort options are available, and the dentist will explain what fits your situation.

How long does it take to heal after an extraction?

Most people feel better within a few days, and the gum usually closes over the next couple of weeks. The bone underneath takes longer to fully heal. Healing time varies with the tooth, your health, and whether you smoke. Follow your aftercare instructions closely.

What is dry socket?

Dry socket happens when the blood clot in the empty socket loosens or dissolves too early, leaving bone exposed. It usually causes throbbing pain a few days after the extraction. Avoid straws, smoking, and forceful spitting to protect the clot. Call us if pain gets worse instead of better.
